Care First Recruitment are recruiting Care Assessors for the Brent area Job Purpose The purpose of this role is to carry out high-quality social care and joint health assessments to identify individuals needs and strengths. The Care Assessor will develop detailed, person-centred support plans that promote independence and wellbeing, ensuring the most appropriate care options are explored, including traditional care, reablement, or supported living. The role involves actively contributing to care discussions and multidisciplinary assessments, using creativity and professional judgement to recommend suitable services. This includes researching and coordinating both formal and informal support options, monitoring and reviewing care packages to ensure they remain effective and responsive to changing needs. The Care Assessor will work collaboratively with statutory, voluntary, and private sector partners, supporting effective partnership working and contributing to the ongoing development of the team.
