Care First Recruitment are recruiting Care Assessors for the Brent area Job Purpose The purpose of this role is to carry out high-quality social care and joint health assessments to identify individuals needs and strengths. The Care Assessor will develop detailed, person-centred support plans that promote independence and wellbeing, ensuring the most appropriate care options are explored, including traditional care, reablement, or supported living. The role involves actively contributing to care discussions and multidisciplinary assessments, using creativity and professional judgement to recommend suitable services. This includes researching and coordinating both formal and informal support options, monitoring and reviewing care packages to ensure they remain effective and responsive to changing needs. The Care Assessor will work collaboratively with statutory, voluntary, and private sector partners, supporting effective partnership working and contributing to the ongoing development of the team. The role also requires taking appropriate action in relation to Safeguarding Adults, in line with local safeguarding policies and procedures. to join our growing team in the Brent area. The successful candidate will be responsible for carrying out care assessments, reviews, and supporting the delivery of high-quality, person-centred care for individuals in their own homes. Key Responsibilities Conduct initial care assessments and risk assessments in service users homes Complete care plans and reviews in line with CQC standards Liaise with service users, families, healthcare professionals, and care staff Ensure care packages are tailored, safe, and outcome-focused Maintain accurate records and documentation Support compliance with safeguarding, health & safety, and company policies Requirements Previous experience as a Care Assessor, Senior Carer, or similar role Achievement of NVQ level 3 or equivalent qualification or academic standard. Experience of working in Social Care Services or similar organisations such as the NHS or Voluntary Sector Strong knowledge of CQC standards and person-centred care Excellent communication and assessment skills Ability to work independently and manage a caseload Good IT and report-writing skills Full UK driving licence and access to a vehicle (preferred) Right to work in the UK
