Tempting Recruitment are working alongside our client, a Local Authority, who are looking to recruit an experienced Social Care Assessor (Adult Social Care) to join their team based in London.

JOB SUMMARY
The purpose of a Social Care Assessor in Adult Social Care is to apply the Care Act 2014 and enhance eligible people’s well-being. The Social Care Assessor will need to apply their social care knowledge and skills to understand and build relationships, and work directly with individuals, their families, and carers in a strengths-based way, to enable and empower them to achieve best outcomes. To work within statutory frameworks to undertake assessments and reviews with adults and carers with less complex needs, where screening does not trigger the need for an assessment by a Social Worker or Occupational Therapist, and to plan care and support making the best use of available resources to enable people to have better lives (Hospital Based).

JOB DESCRIPTION
The duties and responsibilities of the role will include:

To be based in a Hospital discharge team.
To work with adults and carers to develop and agree support plans (including in an emergency), to promote well-being by meeting eligible, unmet needs and make clear informed decisions about the level of support required. To ensure these plans use Council resources to best effect and that authorisation is sought before implementation via the Service Acquisition and Market Management Service.
To complete person-centered reviews of the intended outcomes in the support plan and agree if anything needs to change. To identify if the person’s needs have changed and if a reassessment is required.
THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:

Recognised professional Social Work qualification (Degree/Masters/Post Graduate Diploma/Social Work fast track qualifying programme)
Current Social Work England registration
Experience working in Adult Social Care setting
