We are looking for an experienced and compassionate Senior Care Coordinator to join our growing team and play a key role in delivering exceptional, person-centred care to our clients.

As a Senior Care Coordinator, you will be at the heart of our service — coordinating care schedules, supporting care teams, conducting assessments, and ensuring care plans are delivered to the highest standard.

You will also act as a trusted point of contact for clients, families, and care staff, ensuring communication is clear and that every client receives the support they need to live comfortably and safely at home.

This role also includes occasional hands-on care support (approximately 15–20 hours per week when required) to ensure continuity and quality of care.

Key Responsibilities

Coordinate and manage care schedules to ensure visits are delivered reliably and on time

Conduct care assessments and create or review personalised care plans

*

Act as a key point of contact for care staff, clients, and families

*

Monitor care delivery and respond promptly to changes in client needs

*

Provide guidance, support, and leadership to care staff

*

Maintain accurate records, reports, and care documentation

*

Provide hands-on care support and cover visits when needed to maintain service continuity

About You

We are looking for someone who is organised, proactive, and genuinely passionate about care.

You will have:

Minimum 2 years’ experience in care coordination, senior care, or a similar role

*

Experience conducting care assessments and care planning

*

Excellent organisation and communication skills

*

Ability to manage multiple schedules and priorities effectively

*

Strong IT and administrative skills

*

A full UK driving licence and access to a vehicle

*

A willingness to provide direct care support when required
