Senior Care Coordinator

I'm recruiting for an exciting Senior Care Coordinator opportunity with an award winning domiciliary and live-in care company in Newport. They're looking for someone keen to progress within the company and also provide 1:1 support to their existing Care Coordinator.

What’s on offer:

£28,000-£30,000.
Paid on call (£400 per weekend, one weekend per month only).
Birthday off + 28 days' annual leave.
Funded CPD toward Level 3/Level 5 qualification.
Key Responsibilities:

Managing and updating rotas to meet client needs across both domiciliary and live-in care packages.
Supporting the Care Coordinator on a 1:1 basis, providing guidance and operational oversight.
Handling on-call duties (1 midweek evening and 1 weekend in 4) to maintain continuity of care.
Liaising with clients, families and professionals to ensure high standards of care and satisfaction.
Responding to changes in care requirements, including hospital discharges and urgent staffing needs.
Monitoring compliance, care plans and service delivery in line with company and regulatory standards.
Maintaining accurate records and utilising care systems effectively to support service delivery.
Essential Requirements:

Previous experience as a Care Coordinator within domiciliary care, live-in care or supported living.
Experience completing on-call duties and managing out-of-hours issues.
Strong organisational skills with the ability to prioritise in a fast-paced environment.
Positive and personable, able to work effectively within a busy, open office environment
Confidence using digital care planning and rostering systems.
Strong communication skills, able to build relationships with staff, clients and management.
