Our Clinical team is supported by Pharmacists, Neighbourhood Navigators, Case Managers for frailty and a good community mental health support.Job responsibilitiesThe post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all our patients.Clinical ResponsibilitiesIn accordance with the practice timetable, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations: acute and non-acute, checking and signing repeat prescriptions and dealing with prescription queries.
