The Role

Job Purpose
To provide high-quality, safe, and effective face-to-face clinical care to patients accessing the North & West Kent Out of Hours (OOH) bases . You will assess, diagnose, and treat patients referred via NHS 111, community services, or self-referral, working collaboratively with CAS clinicians, reception and dispatch teams, and local system partners to deliver excellent urgent care and reduce unnecessary hospital attendances .

Hours:
Weekdays: Tunbridge Wells / Maidstone 19:0008:00; Dartford 18:3008:00
Weekends: Tunbridge Wells & Maidstone Friday 19:00Monday 08:00

Key Responsibilities
Provide face-to-face consultations for patients attending OOH bases at Tunbridge Wells, Maidstone, Dartford, and Gravesend
Undertake home visits for patients allocated through IC24 dispatch systems using CLEO on Toughbook device
Undertake comprehensive assessments, diagnostics, treatment, and prescribing in accordance with IC24 clinical guidelines .
Liaise with CAS clinicians and navigators to support effective triage and patient flow.
Prioritise cases based on clinical urgency and ensure safe redirection to alternative services where appropriate .
Communicate effectively with patients, relatives, and healthcare professionals, providing clear safety-netting and follow-up advice .
Maintain high standards of documentation on CLEO, EMIS, or SystmOne systems.
Uphold the principles of patient confidentiality and data protection at all times.
Support audit, clinical governance, and continuous improvement initiatives.
Contribute to peer learning and IC24s weekly educational programme.
