Team Manager Qualified Adult Social Worker Hospital Discharge Location: Cheshire   Pay rate:  £ 40 per hour Our Client is looking for Qualified Adult Social Workers to join Adult Services as a Team Manager for the Hospital Discharge team. You would ideally have 2-3 years’ experience within Adults Social Care. Throughout all that you do you will ensure that you uphold and champion best practise.  Together we focus on solving problems before they escalate and we meet the needs of our vulnerable adults, reducing the risk of harm.
