About The Company This role is required for a dedicated Support Worker to join us, working on our Night Shift. Shift Pattern: 11 hours, between the hours of 20.30PM - 07.30AM.

About The Role As a Support Worker, you will be at the forefront of providing high-quality, enabling care. Pleasant residential bungalow with garden, within the local community, offering care & personalised opportunities for each individual. Key Responsibilities: - Individual Outcome and Wellbeing: Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance. - Activity Planning: Develop flexible, engaging activity plans that promote autonomy and personal growth. - Health and Lifestyle: Support individuals in maintaining healthy lifestyles and managing their health needs. - Communication: Facilitate effective communication and maintain detailed, person-centered records. - Medication Management: Oversee medication processes and support individuals in self-administration. - Safety and Environment: Maintain a safe, clean-living environment and adhere to health and safety protocols.
