Social Care Assessor (Hospital Discharge Team)
Location: Enfield
Contract: Initial 3 months
Hours: 36 hours per week
Pay: £23.87 per hour (Umbrella) / £18.95 per hour (PAYE exclusive)
About the Role
Enfield Council is seeking an experienced Social Care Assessor to join our Hospital Discharge Team (HDT). This is a fast-paced, ward-based role supporting the safe, timely, and effective discharge of residents from hospital into appropriate care and support settings.
You will work closely with NHS colleagues, families, and social care professionals to ensure smooth discharge pathways and positive outcomes for service users.
Key Responsibilities
Act as a first point of contact for adult social care within hospital settings
Undertake proportionate Care Act assessments and eligibility decisions
Identify and allocate appropriate care and support resources
Coordinate care packages to support timely hospital discharges
Work collaboratively with NHS staff, hospital wards, families, and providers
Manage and reduce discharge delays through practical problem-solving
Ensure clear, outcome-focused recording and documentation
Provide effective handovers to longer-term social care teams
Maintain accurate records in line with data protection requirements
Contribute to service improvement and team developmentAbout the Team
You will join a supportive and well-structured Hospital Discharge Team with strong management oversight and peer support. The team plays a key role in enabling safe discharge pathways across Enfield’s hospital settings.
About You We are looking for someone who:
Has experience in adult social care assessment, ideally within hospital discharge or similar fast-paced environments
Can make confident, proportionate decisions under pressure
Has strong communication and partnership-working skills
Understands the Care Act and safeguarding responsibilities
Can manage competing priorities and fluctuating workloads
Is committed to delivering person-centred, strengths-based practiceAdditional Information
Hybrid working available, with regular on-site presence required
Work involves supporting vulnerable adults and liaising with health and community partners
