Introduction

Acorn by Synergie is recruiting for a Senior Support Worker to join a team in Newport. This role involves supporting individuals in their own homes by delivering high-quality, person-centred care while also providing leadership and guidance to a team of support workers.

The successful candidate will help ensure service users receive safe, dignified and personalised care that promotes independence and wellbeing, while supporting the supervision of staff and maintaining care standards.

Key Duties:

Provide person-centred care to service users in their own homes, promoting independence, dignity and wellbeing. Support individuals with daily living activities in line with their care plans. Encourage and facilitate social interaction and participation in community-based activities where appropriate. Monitor service users' health and wellbeing and report any changes or concerns to the Care Manager or relevant professionals. Assist with medication administration and ensure accurate documentation of medication records. Supervise, mentor and support a team of support workers to maintain high standards of care.
