Derbyshire Dales Short Term Assessment and Reablement Team (START)
There is an exciting opportunity for a Senior Practitioner to join the Derbyshire Dales Short Term Assessment and Reablement Team (START) based at Shand House, Darley Dale. This is a fast-paced and highly specialist team who are focused on the wellbeing, empowerment and reablement of people in our community. Wherever possible we aim to prevent the need for hospital admissions and entry into longer term care settings and enable people that are stepping down following a hospital stay, helping them to achieve their full potential to live as safely, independently and as well as possible in their own homes.
We are seeking a skilled and motivated Senior Practitioner to help develop our team and oversee, manage, and advise on some of our more complex work. This is a new team, and this post offers an opportunity to contribute to and develop systems and processes to improve the efficacy of our interventions and promote the independence and wellbeing of the people of Derbyshire Dales. We take a multi-disciplinary approach, and the team is a mixture of Social Workers, Social Care Practitioners, Occupational Therapists, Enablement Leads and Enablement Support Workers. We work closely alongside our health colleagues, including running our START DUTY at The Whitworth Hospital, Matlock (and other integrated co-locations).
Experience of managing a variety of Care Act (2014) interventions with a focus on personalised strength-based approaches and outcomes is essential. You will offer support, advice, and professional supervision to all members of the team. You will produce performance reports, chair meetings, and deputise for the Service Manager when required. You will have an overview of Duty and will be overseeing and advising on S42 safeguarding enquiries, VARM, Mental Capacity (2005) assessments and Best Interest Decisions.

We offer hybrid working, but there is an expectation that you will be regularly co-located with Duty workers and providing face to face support to the team
