Team Lead (Primary Care Network Service)

Responsible to: Service Manager (Complex Needs & Community)
Working hours: 37.5 Hours per week (Monday – Friday, 9am – 5pm)
Rate of pay: £31,000 - £32,000 per annum
Working base: Hemel, Watford or Boreham Wood
Area covered: The role requires travel across Hertfordshire to our Wellbeing centres, community partnerships and external organisations

For over 50 years, we have supported the people of Hertfordshire with their mental health. We offer a range of Community Support services which provide advice, information, onward referral and holistic outreach support to people who are experiencing mental ill-health or who need help with their mental wellbeing. Our high quality services are flexible and we help people to resolve real-life difficulties and to improve their own independence, quality of life and wellbeing. Each year, we help 15,000 people experiencing mental ill health.
We are a local Mind, affiliated to national Mind, the leading mental health charity in England and Wales. This means that we are an independent charity responsible for raising our own funds through contracts, grants and fundraising.
About our Primary Care Network Service
The Primary Care Network Service works in partnership with GP surgeries in Hertfordshire to provide advice, information, onward referral and holistic support to individuals who are experiencing mental ill health or need support with their mental wellbeing.
About the role
The Team Lead plays a significant role in the co-ordination and running of the Primary Care Network Service. This role is key to the development and support of the team and the wider organisation and will be part of the new NHS Neighbourhood plans.
Responsibilities of the post
* To line manage a team of Community Support Workers.
* To provide monthly supervision to support client work and staff wellbeing.
* To be responsible for the Primary Care Network Service quarterly reporting.
* To support the team in doing high quality case studies.
* To participate in the recruitment of new Community Support Workers.
* To carry out HMN’s induction program for new Community Support Workers.
* To ensure Community Support Workers receive regular and up to date training to support their job role and well-being.
* To build strong relationships with GP surgeries across Hertfordshire.
* Ensure a direct community service is in place which encourages, motivates and builds patients self-esteem and confidence. This will include goal setting and support with social and practical skills development.
* To undertake initial contact with clients and complete a holistic needs assessment, making use of assertive outreach work to achieve the initial client contact, if required.
* To assist clients to access appropriate mental health services, drug and alcohol services, housing/tenancy services and other services according to assessed needs.
* To provide crisis/safeguarding intervention support. This will include responding to problems, which pose an immediate risk.
* Support clients to build their emotional resilience and crisis planning tools, promoting self-management through building an understanding of their own triggers/coping strategies.
* To provide advocacy where requested by clients, accompanying them and ensuring their voice is heard when communicating with other agencies.

Being able to drive and having access to your own vehicle (or equivalent) is essential for this role.
