PCN Home Visiting Nurse (ACP)

We have a vacancy at West Leeds PCN for a prescribing Nurse or Nurse Practitioner. This role would particularly suit an Advanced Practitioner with good all-round nursing experience, as the role itself will be very broad. We need someone who is an experienced Independent Prescriber, particularly for the Acute Home Visiting element of this role. You would be joining a highly skilled and experienced team with a wealth of peer support available each day.
Main duties of the job
There are two elements to this role:
- Annual frailty reviews - These are templated and in-depth. We focus mainly on those that are severely frail but also work with moderately frail patients to improve health outcomes and prevent deterioration. These reviews are primarily conducted in patients' homes unless they are able to visit the surgery.
- Acute home visits - You will spend part of your week conducting these visits, which help relieve pressure on member practices, especially GPs.

Job responsibilities
Our Frailty Service is multidisciplinary, focusing on patients over 65 who demonstrate two or more frailty indicators (e.g., falls, immobility, incontinence, delirium, medication side effects). Patients are assessed at home or in care homes, and a bespoke care plan is created to improve or maintain their health and independence. Each referral is considered an intervention, and after review, patient care is handed back to their GP practice.
Patients are assessed for falls and medication review. An independent prescriber would be advantageous but is not essential, as other team members can issue medication.
You will attend weekly MDT meetings with a consultant geriatrician to discuss complex cases and will liaise with the wider PCN team to deliver holistic healthcare. The successful candidate should be passionate about improving patient health outcomes.
The daily Acute Home Visiting list offers variety and includes issues such as swellings, UTIs, chest infections, oedema, wounds, skin rashes, and migraines. This service is vital for our practices and helps free up GP time.

Salary: £46,000 to £55,000 per year, depending on experience.
