Job responsibilities

We are seeking a motivated and compassionate Nurse Practitioner to join our team. The ideal candidate will be responsible for providing comprehensive care to our patients, working collaboratively with our multidisciplinary team.

Key Responsibilities

• Practice autonomously within scope using a person-centred, partnership-based approach.
• Assess and diagnose individuals accurately during acute and long-term health episodes.
• Gather and evaluate clinical data to make informed, evidence-based decisions. Carrying out referrals if necessary.
• Coordinate and manage care plans to optimise health outcomes and resource use.
• Provide emotional support and reassurance to individuals, carers, and families.
• Order, carry out and interpret tests such as blood work, imaging, or screenings to support diagnosis and treatment planning.
• Apply sound clinical judgment and use up-to-date research to guide decisions and care.
• Deliver health promotion and disease prevention advice, including support for smoking cessation, weight management, and contraception.
• Manage long-term conditions by providing patient education.

Person Specification

Qualifications

• Registered with the Nursing and Midwifery Council (NMC) to practice as a Registered Nurse
• Completed an NMC-approved nursing degree (BSc or Nursing Degree Apprenticeship)
• Evidence of a commitment to on-going personal & professional development
• Postgraduate Diploma or Certificate in a Specialized Area
• Teaching and Mentorship Qualification

Experience

• Experience working as a Nurse Practitioner in a primary care setting
• SystmOne Experience
• Excellent communication and interpersonal skills
• Ability to work effectively as part of a multidisciplinary team
• Knowledge of current NHS policies and procedures
• Interest in Women's Health
