About The Role Salary: £30,471.40per annum (£15.79 per hour) Contract: Permanent Hours: Full time, 37 hours per week Shifts: 5 days over 7 on a rolling patternbetween 9am 7pm including weekends Location: Travel required across Barking & Dagenham. Base in Brentwood.

As a key member of the team, utilising your care expertise, you will: Engage with customers to maximise their independence and achieve individualised goals Conduct initial assessments with our customers to assess their needs and develop support plans Work closely with our team of health and social care workers, and external health professionals, to maximise the support available to customers
