About the Role We are looking for an experienced Occupational Therapist to join our short-term, therapist-led reablement service. This role focuses on supporting individuals to achieve their goals, reduce the need for ongoing care, and live as independently as possible. You will work closely with customers, carers, and multi-disciplinary teams to design creative support plans and deliver targeted interventions.
Key Responsibilities Undertake strengths-based occupational therapy assessments for customers with complex needs.
Design and implement innovative reablement plans to promote independence and reduce long-term care requirements.
Carry out complex moving and handling risk assessments and recommend adaptations.
Coordinate the customers reablement journey, liaising with families, health professionals, and support staff.
Provide professional supervision and support to colleagues within the team.
Ensure safeguarding responsibilities are met in line with legislation and local policy.
