We are looking to recruit a Deputy Manager - The Reablement and Hospital Discharge Team within Milton Keynes County Council.

Responsibilities include:

Working with Vulnerable Adults within a hospital setting Report writing and making assessments Developing care plans Care management Managing complex cases Liaising with other professional bodies Arranging safe and timely discharges for service users Self-directed support / personal budgets Managing staff Provide reflective supervision and support to social workers and social work assistants Manage and prioritize allocations based on risk, need, and urgency. Support workers in producing strengths-based assessments that accurately reflect the individual and their situation. Collaborate with health colleagues and other professionals to ensure timely discharge of individuals. Oversee the flow through all discharge services. Manage and support a team, addressing HR issues as necessary. Support colleagues within a multidisciplinary service. Solve problems and manage complex cases and risk.

Experience preferred:

Post qualifying experience of working as a Senior Social Worker - Hospital Discharge team within the UK Social Care industry.
