Responsibilities:

- Be the first point of contact for patients and provide high-quality patient care in-home, ensuring the comfort and safety of those under your care - Administer medications accurately and in accordance with NMC guidelines. - Prepare and maintain patient records including care plans accurately and effectively, and ensuring legal requirements are complied with in an ethical manner - Assess, understand and monitor patients' welfare by discussing their conditions with them and providing access to therapeutic activities - Conduct risk assessments and monitor progress with the family
