They determine what we do, how we do it and what we are trying to achieve.n-compass is a Great Place to work and the successful applicant would be joining an organisation where our people come first and we invest heavily in providing a rewarding and fulfilling employment experience.Join Our Team and Make a Real Difference!To provide Carers with high-quality information, personalised assessments, contingency planning support and access to the services that will reduce the impact of their caring responsibilities.This role is all about helping Carers understand their rights, get the support they need, and feel confident in their caring roles.Key ResponsibilitiesIn this vital role, you will: Contact newly referred Carers, welcome them to the service and explore their initial needs.Carry out Carers Assessmentsusing each Carers preferred methodwithin agreed timescales.Use a whole-family approach: involving those the Carer chooses, and identifying others (including young Carers) who may also need support.Identify whether a Carer has difficulty engaging in the assessment (as defined by the Care Act 2014) and arrange advocate support if needed.Determine Carer eligibility using National Eligibility Criteria.For eligible Carers, co-produce proportionate support plans that directly address their assessed needs.Submit support plans and budget recommendations to the Council when required, explain outcomes, and ensure families understand the process.Complete a light touch review within 8 weeks to check the plan is achieving the Carers expected outcomes and gather feedback.Provide non-eligible Carers with information, advice, signposting and preventative support options.Ensure assessments and support plans meet the Carers Assessment & Support Planning Quality Assurance Framework.Provide up-to-date information on Carers rights and available services.Tailor support for Carers with specific needsincluding mental health, learning disabilities and dementia.Make appropriate referrals to community services and organisations.Support Carers in completing contingency plans to outline emergency arrangements.Maintain accurate, high-standard case recording on both Local Authority and internal systems.Contribute to reporting through high-quality written records.Meet performance indicators set by your Team Leader.Promote and safeguard the welfare of adults and children.Undertake Review Officer duties when required and support the wider Carers Service.Signpost and encourage Carers wishing to develop their own peer groups or activities.Help build a living directory of local wraparound support.Support the induction and development of social work students and volunteers.Please find attached on our website, the full Job Description, Person Specification and benefits package.Successful applicants will be required to undergo an enhanced DBS check.n-compass is an equal opportunities employer.
