At DHU Healthcare CIC , our Primary Care teams work at the heart of local communities, delivering high-quality, patient-centred services across the areas we serve. Based at Eyam Surgery, with flexibility to support Bradwell Surgery when required, you'll play a vital role in ensuring patients receive timely access to the right care and a positive experience from the moment they contact or visit the practice.
Our teams of skilled clinicians and support professionals work together to provide responsive, professional NHS services. As a Care Navigator, you'll be central to guiding patients to the most appropriate service, helping the practice run efficiently while always keeping patient wellbeing at the forefront.
As a Care Navigator you will:You'll bring excellent communication and interpersonal skills, strong organisational ability and confidence using IT and digital systems. You'll be calm under pressure, able to multitask in a busy environment and committed to delivering a patient-focused, compassionate service.
