Position: Care Coordinator Location:Partington, Greater Manchester Reports To: Home Manager Role Overview: As a Care Coordinator, you will play a vital role in delivering high-quality, person-centred care to our residents. You will work closely with the care team to ensure the coordination and management of care plans tailored to each resident's needs, preferences, and goals. Your responsibilities will include shift organisation, medication administration and recording, liaising with healthcare professionals, and maintaining up-to-date care plans.
