Job Introduction This exciting new development within Lincolnshire, based in Grantham, welcomes enthusiastic and experienced applicants to apply for this rare opportunity to be involved in shaping a brand new pilot service that will support individuals stepping down from acute mental health wards to transition into the community. The service will be delivered in partnership with Lincolnshire Partnership NHS Trust and will form part of the NHS Mental Health Pathway. The service will be going live in October 2025, with a full training and induction programme for the team, commencing prior to go live.
