We are looking for an Advanced CarePractitioner Team Leader (Advanced Nurse Practitioner / Paramedic), who canprescribe, to join our already established Home Visiting and Care Home team.The practitioner will provide on the day acute visits for housebound patients andcare home residents within the PCN, as well as delivering proactive visits tosupport the ongoing health needs of our population.This post holder will be the teamleader for the current Home Visiting team which consists of 2 prescribing ACPsfrom a nursing background and a paramedic. The team is likely to expand in thefuture. They will provide clinical and operational leadership for the PCNservice, ensuring safe, effective, and person-centred care to houseboundpatients & care home residents across Southampton West PCN. This rolecombines direct patient care with key leadership responsibilities includingstaff supervision, rota coordination, governance, and service development.
The post holder will be required to provide on the dayvisits for housebound patients who are acutely unwell and need a clinicalassessment, as well as pre-planned proactive care assessments. The practitionerwill also need to visit patients living in care homes for both acute andproactive care, this will be in the form of either a ward round or an acutevisit. The clinician will be responsible for prioritising visits according toclinical need and will be required to take a comprehensive history and devise atreatment plan.
Main duties of the job
As part of the proactive care service delivery, the postholder will be required to complete comprehensive frailty assessments, advancedcare plans, end of life care planning, and medication optimisation whileworking closely with a wider MDT to identify patients who require input fromother services or agencies.
The post holder will work with PCNmember practices and the PCN leadership team to continuously develop theservices, this will include reviewing and developing clinical policy, practice,and procedures to ensure the service is safe, efficient, and effective for our houseboundand care home residents and GP member practices.Job summary

The post holder will have access to a wide range of supportfrom the PCN and Member Practices, this includes regular supervision with a GP.

Primary Duties as Team Leader
Clinical Leadership
- Ensure the Home Visiting Team deliver high-qualityclinical care, including assessment, diagnosis, treatment, and prescribing, atall times.
- Support complex clinical decision-making.
- Lead on clinical supervision, medication safety, andprescription audits with input from frailty GP and pharmacists.
- Provide support with clinical queries and escalation.
TeamManagement
Overseedaily service operations and rota coordination for the AVS team working closelywith the Care Coordinators and PCN Leadership Team.
Manageteam development including:
- Monthly 1:1s and clinical supervision
- Probation reviews
- Annual appraisals and performance management
- Lead on recruitment and induction of new team members.
- Lead on team clinical development and training
- Ensure compliance with mandatory training and clinicalcompetencies.
- Foster a supportive and inclusive team culture.

Service& Strategic Development
- Support development and implementation of HomeVisiting Service protocols and policies.
- Work collaboratively with the PCN Business Manager,Frailty GP Lead, and Clinical Directors to enhance service delivery.

Primary Duties as an Advanced Care Practitioner
- Uses expert knowledgeand clinical skills to deliver holistic care to housebound patients and carehome residents registered with Southampton West PCN
- Provide safe, evidence-based,cost effective, individualised patient care
- Assesses diagnoses andtreats patients attending primary health care services with a range of acute,non-acute, and chronic medical conditions
