Role Purpose To provide a high quality and effective social work service, which responds to the needs of people with lived experiences with care and support needs, their informal carers and their families in a timely manner and within statutory guidelines. All assessments and support plans will be completed in accordance with the wellbeing principles of the Care Act 2014 and reflects a strengths-based approach.
