Adult Social Worker - Hospital Discharge Team Brent Council

Brent Council is looking for an experienced Adult Social Worker to join their Hospital Discharge Team. This is a great opportunity to make a real difference by supporting safe and timely discharges from hospital.

Working Hours: Monday to Friday 09:00 - 17:00

Pay Rate: Up to £32.00 per hour (Umbrella)

Role Overview: As part of the Hospital Discharge Team, you will:

Support adults to be discharged safely from hospital

Complete assessments under the Care Act 2014

Work closely with health professionals, families, and partner agencies

Arrange appropriate care and support packages

Promote independence and positive outcomes for service users
