Adult Social Worker, Locality Team in Enfield

The post is a level 3 Social Work position within Adult Social Care - East Locality Team, focusing on clients who are Older People, Adults with Disabilities, and Vulnerable Adults. Key responsibilities include:

1. Conducting Care Act Assessments, Support Planning, and Reviews.
2. Handling CHC Assessments, Asylum/NRPF cases, Transition, and Safeguarding.
3. Undertaking comprehensive social care assessments and producing detailed reports and support plans.
4. Applying knowledge of Direct Payments using a strength-based approach.
5. Understanding the Continuing Healthcare process, legislation, and Decision Support Tools.
6. Communicating sensitively with family members of service users.

Work arrangements are flexible, including face-to-face visits/assessments, work from home, and office-based work as needed.

Rate: £31.04 per hour
