We are seeking an experienced and skilled Advanced Social Worker (ASW) to join our team in London. As an ASW, you will be working in a multi-disciplinary setting, with a focus on strengths-based practice and promoting independence and recovery. This is an exciting opportunity to work with a diverse range of individuals with complex social needs, including those with varied health and social care needs.
Candidate Responsibilities:
Complete Care Act assessments and devise care and support plans Conduct reviews and Mental Capacity Assessments, chairing Best Interests meetings as required Provide crisis intervention, signposting, and advice and information to service users and their families Work closely with individuals, including informal carers, to provide comprehensive support tailored to their needs Facilitate Best Interests meetings and deliver timely signposting to necessary community resources Complete initial fact finding and assess and minimise risk within the Safeguarding function Benefits: ...
