East Lothian Health and Social Care Partnership is seeking two experienced and motivated Social Workers to join our newly established Prevention and Early Intervention multi-disciplinary team.
This innovative team works collaboratively with NHS colleagues and partners from Community First, hospital flow and discharge services, reablement, social work, and care-at-home teams. Prevent unnecessary hospital admissions

As a Social Worker in this team, you will:
Carry out assessments for individuals in the community
Deliver responsive, person-centred support to enable independence and recovery
Currently Social Work element of this service operates Monday to Friday. There is the Potential for future seven-day working, with weekend shifts anticipated up to two weekends per month.
While eligible for the Council’s flexible working scheme, the nature of this role demands a high level of agility and responsiveness. You will also have the chance to work with the wider Social Work Team at John Muir House, offering opportunities to broaden your practice and develop within the wider context of Adult Social Work.
Home / Hybrid Working Consideration

All home/hybrid workers must live in the UK. Where an application for home-working is supported, it will be subject to a satisfactory trial period. Employees with a hybrid working arrangement in place will be office based and able to work from home, with permission, on an ad hoc basis.
Job Details To provide an effective service to service users and their families/carers, promoting a high standard of practice, in accordance with statutory obligations, policies and procedures.
Approved qualification in Social Work, and SSSC registration.
Those with international Social Work qualifications should not apply unless they are fully registered with the SSSC.
