Wearside Medical Practice is a hardworking, friendly profitable over 8000 patient practice close to Sunderland city centre. Situated within a purpose built building. We are adjacent to Sunderland Royal hospital . Good CQC ( June 2022) and high achieving QOFWe are looking to expand and develop our Practice and to do this we are looking for a talented, forward thinking and committed GP who will be able to provide support and lead the practice .This salaried post is open to a newly qualified GP and experienced GP.The practice would support candidates who require TIER 2 VISAWe are looking for a friendly enthusiastic person to compliment our established team, who will passionate ,driven and with a desire to make a difference to the practice and the communityYou will be supported by a highly experienced administrative and clinical teamEMIS webSalary dependant on experience6 weeks leaveProtected time if wanting to become a GP trainerPart of PCN- Supported by PCN Pharmacists , Social Prescribers , Dietician, Mental Health Practitioners . To undertake clinical sessions ( routine and on call) followed by a variety of duties such as queries,home visits,checking and signing repeat prescriptions, actioning test results,medical reports and examinations. The appointee will also have the opportunity to take part in leading on key disease areas, staff training and monitoring trends within QOF.In addition attend MDT, clinical, safeguarding, significant events, and provide care for patients within our aligned care homeActively participate in the the startegic review, audit and development of the practice, ensuring improvement of quality health outcomesAbout usWe constantly strive to ensure our patients receive an excellent patient experience. We have an established and fully complementary administrative and clinical team to support this role, including 3 ANP's, 2 PN'S , and 2 HCA.The practice is supported by a PCN pharmacist and mental health practitioner .We provide care for 55 patients in our aligned care homeJob responsibilitiesMain purposeTomanage a busy and interesting clinical caseload. The appointee will also havethe opportunity to take part in leading on key disease areas (monitoring trendsin the Quality and Outcomes Framework, treatment, staff training andprescribing) and assist in developing appropriate healthcare services for adiverse, patient list (commissioning, patient participation and healthpromotion).ClinicalresponsibilitiesInaccordance with the practice timetable, as agreed, the post-holder will makehim/her-self available to undertake clinical sessions (routine and on-call). Aclinical session is normally composed of 12 13 appointments , followed by a variety of duties includingqueries, visiting patients at home, checking and signing repeat prescriptions,actioning test results, medical reports and examinations (e.g. InsuranceCompanies) on behalf of the Practice. The above duties should be performed in atimely manner using practice protocols.Appointmentswill be 10 minutes durationYou will berequired to:Act upon alerts (e.g. QOF) and remindersduring a routine consultation i.e. (10 minutes)Provide all GP services required as part ofthe GMS contractUndertake surgeries during extended hours inaccordance with the access needs of the practice population and the contractualobligation of the practiceProvide cross cover, where necessaryRespond to medical problems presented bypatients, including history taking, examination, investigation, diagnosis,treatment and referral where appropriateProvide appropriate preventative health careand adviceProvide medical support as required topatients referred by nursing staffAdhere to the practice prescribing formulary,local and national guidelinesMake professional, autonomous decisions inrelation to presenting problems, whether self-referred or referred from otherhealth care workers within the organisationRecord clear and concurrent I.T. basedconsultation notes to agreed standardsCollect data for audit purposesCompile and issue computer-generated acuteand repeat prescriptions (avoiding hand-written prescriptions wheneverpossible.Other responsibilitieswith the PracticeEngaging in appropriate lifelong educationaland audit activity in order to maintain clinical competence and performance,including the completion of objectives agreed by yourself and the practice, inan annual Personal Development Plan.Actively participating in the training anddevelopment of nurses, medical students and GP trainees in the practiceSupporting the practice staff and respondingto requests for advice and assistance from the practice reception, secretarial,administrative and nursing staffAttend prescribing, palliative care, clinicaland practice ,safe guarding meetingsYou will be expected to take part in clinicalmeetings, You will receivefeedback via peer review and are required to act upon recommendations anddemonstrate change.Meet all tight timescales/deadlines foraudits and written returns to ensure that the Practice meets quality standardsand receives the designated funding (e.g. Quarterly Enhanced Services returns,annual QOF audit etc.)General DutiesTheabove responsibilities will be undertaken upholding the following:To continue to work until all clinical taskshave been completed. To make arrangements with others to cover when planning toleave before the end of the sessionTo maintain the financial security of theorganisation by ensuring accurate and efficient responses to income generationsuch as reports, dispensed prescriptions, private forms etc.Identifying the risks involved in workactivities and undertaking such activities in a way that manages those risks;Making effective use of training to updateknowledge and skills;Using appropriate infection controlprocedures, maintaining work areas in a tidy and safe way and free fromhazards;Personal/ProfessionalDevelopmentInaddition to maintaining continued education through attendance at any coursesand/or study days necessary to ensure that professional developmentrequirements are met, the post-holder will participate in any trainingprogramme implemented by the practice as part of this employment, such trainingto include:Participation in an annual individualperformance review at practice level and an external annual NHS GP Appraisalincluding taking responsibility for maintaining a record of their own personaland/or professional development. Where there is no separate and distinctpayment by the CCG/NHS, the Employer is under no obligation to make a separatepayment. Any time preparing for the appraisal or conducting the appraisal maybetaken from the employees annual study leave entitlement, with prior agreementTaking responsibility for own development,learning and performance and demonstrating skills and activities to others whoare undertaking similar work.Maintain current CPR and anaphylaxiscertificationQualityThepost-holder will strive to maintain quality within the practice, and will:Participate in clinical governance activityand contribute to the improvement in quality of health outcomes through auditand the Quality and Outcomes FrameworkActively participate in the Strategic Review,Audit and Development of the PracticeAlert other team members to issues ofClinical Governance issues, quality and risk; participate in Significant EventAnalysis reviewsAssess own performance and takeaccountability for own actions, either directly or under supervision;Contribute to the effectiveness of the teamby reflecting on own and team activities and making suggestions on ways toimprove and enhance the teams performance;Work effectively with individuals in otheragencies to meet patients needs;Effectively manage own time, workload andresources.
