As Nurse Case Manager, you’ll take ownership of complex care packages for adult clients, ensuring safe, high-quality delivery while developing and supporting care teams.
Key responsibilities:
Managing and reviewing complex care packages in the community Leading clinical quality, governance, and best practice Delivering specialist training (e.g. tracheostomy, ventilation) Assessing and signing off staff competencies Coaching and mentoring care teams Building strong relationships with clients, families, and MDTsAbout You Registered Nurse with NMC PIN Experience in complex care (e.g. tracheostomy, ventilation, spinal injury) Passion for developing others and improving care standards Strong communication and organisat...
