Our mission is to provide high-quality, person-centred support through coordinated, multi-agency approaches.\n Role Overview:\n We are seeking a committed and organised Complex Needs Support Officer to join our office-based team. This role involves working directly with individuals to assess needs, coordinate support, and maintain accurate records, ensuring that service users receive holistic, timely, and effective assistance.\n Key Responsibilities:\n\nConduct assessments and develop person-centred support plans for adults with complex needs.\nProvide guidance and support to service users, including welfare, housing, and community resources.\nMonitor progress and liaise with internal teams and external agencies to ensure coordinated care.\nMaintain detailed and accurate records in line with data protection and organisational policies.\nSupport safeguarding and risk management processes.\nAttend team meetings, case reviews, and contribute to service improvement initiatives.Person Specification:\n\nExperience working with adults with complex needs, including mental health, homelessness, or social vulnerabilities.\nStrong organisational and record-keeping skills.\nAbility to work collaboratively in a multi-agency environment.\nKnowledge of safeguarding, housing, and welfare systems.\nExcellent communication skills and a compassionate, professional approach.If you have the relevent skills then please apply today
