Were partnering with a leading and rapidly growing healthcare provider to recruit a Community Nurse specialising in peritoneal dialysis. This role offers the opportunity to deliver truly personalised care, supporting patients to remain safely and confidently in their own homes. Your caseload will cover the West London region. Youll work closely with patients on a one-to-one basis, giving you the time and autonomy to deliver the standard of care you were trained to provide. Whether youre already in the community or looking to transition into homecare, youll be supported with full specialist training and a highly experienced renal team. The RoleYoull provide specialist peritoneal dialysis care to patients in their homes, playing a key role in maintaining their independence and quality of life. Your responsibilities will include:Delivering Automated and Manual Peritoneal DialysisConnecting and disconnecting dialysis equipment during home visitsCoordinating with hospital renal units and wider MDTsEducating and supporting patients and families in their own environmentThis is a role where you can build genuine relationships and see the long-term impact of your care. What Were Looking ForRegistered Nurse (RGN or RSCN) with active NMC registrationMinimum 2 years post-registration experienceFull UK driving licence and willingness to travelConfident working independently in a community settingPassion for patient-centred, home-based careCommunity experience is welcomed but not essential asfull training is provided.
