*Role Overview:*
Pathways CIC, in partnership with Everybody Health & Leisure, are recruiting for a Community Link Worker (CLW) to join our team in delivering Cheshire East's One You Service. This fully funded, local healthy living service aims to support residents in Cheshire East in starting and maintaining a healthier lifestyle.
As a CLW, you will play a pivotal role in supporting residents to access the One You Cheshire East service, particularly those facing health inequalities such as individuals from deprived areas or those with diabetes and/or hypertension. You will be tasked with connecting residents to One You Services and other community and statutory resources through an asset-based community development approach. The service has a no wrong door approach, and where the service is not appropriate for the resident you will signpost to other community services and provisions to support their needs.
*Key Responsibilities:*
* *Engagement & Empowerment:* Identify and engage Cheshire East residents, especially those with health behaviours such as smoking, drinking, or lack of exercise. Empower them to access One You Services and support them in achieving lifestyle changes.
* *Collaboration:* Work closely with partner organisations, Care Communities, and VCFSE groups to maximise engagement with One You services and ensure a "no wrong door" approach.
* *Referral & Support:* Receive and act upon referrals from the lifestyle team where the service is not appropriate for residents, developing personalised action plans that recognise and support the needs of those referred
* *Community Connection:* Introduce or reconnect individuals to local community groups and services. Build relationships with residents, focusing on their priorities and holistic needs.
* *Reporting & Analysis:* Record referrals, complete case management notes, and analyse trends to ensure effective support for CORE20Plus5 patients and those with identified health behaviours
*Competencies Required:*
* Experience of working in communities / community development
* Strong interpersonal skills with the ability to build trusting relationships.
* Knowledge of health inequalities and social determinants of health.
* Ability to work flexibly, including evenings and weekends as needed.
* Competence in effective case management, data analysis and report writing, including analysis of trends/themes and providing case studies.
* IT literate, proficient in the use of different software such as cloud-based systems including CRM systems, word documents, spreadsheets, PowerPoint.
