Case Manager – Individual Commissioning Learning Difficulties NHS West Midlands, England, United Kingdom Job Summary The case management team is responsible for case management coordination for patients with learning disabilities, mental health, physical health and complex needs. Patient's needs are met and regularly reviewed in line with national requirements for those eligible for NHS Continuing Healthcare (CHC), working within the Shropshire, Telford and Wrekin ICB. As a case manager, you will need a qualification as a registered nurse or social worker, current NMC or HCPC registration and a background working in LD. You must be able to travel within the locality, with occasional requirement for travel out of county. Understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care, Mental Health Act, DOLS, Court of Protection matters is desirable. Ideally, you will have experience as a CHC assessor and commissioner, but prior case management experience is also considered. Main Duties of the Job • Background in patient assessment, liaison with multidisciplinary team, knowledge of care provision. • Observe and evaluate care needs and signpost to appropriate care. • Make a positive difference, monitor quality and assurance and financial agreements. • Develop clinical knowledge of complex care and conditions. • Attend national and local forums to contribute to NHS change and development. • Develop leadership, organisational, IT and digital skills using existing care recording systems. Person Specification Essential
Significant experience in a relevant clinical field. Clear communicator, excellent writing, report writing and presentation skills. Ability to analyse and interpret information, pre‑empt and evaluate issues, and recommend appropriate actions. Decision‑making on difficult and contentious issues. Ability to work independently, organise own workload without supervision. Strong desire to improve performance and achieve goals. Experience of assessment and commissioning (writing care plans). Registered with NMC/HCPC.
Desirable
Understanding of the health and social care environment. Commissioning experience. Knowledge of MCA/BI/DOLS/COP.
