Essential (own vehicle required) An opportunity has arisen for a dedicated Care Coordinator to support the delivery of high-quality, person-centred care across residential and community services. You will work with young people and adults with complex needs, including learning disabilities, autism, physical disabilities, and mental health needs. This is a fast-paced role where you will coordinate care packages, support staff teams, and ensure services meet regulatory standards. Key Responsibilities Coordinate care delivery and maintain continuity of care Manage staff rotas, availability, and cover Support recruitment, onboarding, and staff development Monitor compliance with CQC regulations and internal policies Maintain care plans, risk assessments, and accurate records Liaise with families, professionals, and internal teams Participate in the out-of-hours on-call rota Requirements Experience in complex care or supported living Level 3 Health & Social Care (or willingness to work towards) Knowledge of relevant legislation and CQC standards Strong organisational and communication skills Ability to work under pressure Full UK driving licence and access to a vehicle Whats on Offer 28 days annual leave (including bank holidays) Ongoing training and development Supportive team environment Opportunities for career progression A great opportunity for someone looking to develop their career in a rewarding and impactful role.
